Transcript Request

Current and Alumni/Graduates of New Urban High School can request an unofficial or official copy of their transcript. 

For New Urban Online Transcripts, please visit www.nuo.nclack.k12.or.us

• You will need to provide proof of identity, such as a driver's license, a passport, a state/government identification card with a picture, or a New Urban High School student ID card with a student ID number on it.
• Please submit the Transcript Request form below and the office staff will process it in the next 3 business days.
